Miller, Yafell Serulle, Dheeraj GandhiTeaser Tinnitus is a common symptom that usually originates in the middle ear. Vascular causes of pulsatile tinnitus are categorized by the location of the source of the noise within the cerebral-cervical vasculature: arterial, arteriovenous, and venous. Arterial stenosis secondary to atherosclerotic disease or dissection, arterial anatomic variants at the skull base, and vascular skull base tumors are some of the more common causes of arterial and arteriovenous pulsatile tinnitus. Noninvasive imaging is indicated to evaluate for possible causes of pulsatile tinnitus, and should be followed by catheter angiography if there is a st…

EisenmanTeaser The clinical evaluation of patients with tinnitus differs based on whether the tinnitus is subjective or objective. Subjective tinnitus is usually associated with a hearing loss, and therefore, the clinical evaluation is focused on an otologic and audiologic evaluation with adjunct imaging/tests as necessary. Objective tinnitus is divided into perception of an abnormal somatosound or abnormal perception of a normal somatosound. The distinction between these categories is usually possible based on a history, physical examination, and audiogram, leading to directed imaging to identify the underlying abnormality. (Source:…

Ménière’s disease is a disorder of the inner ear that causes attacks of vertigo and hearing loss, tinnitus, aural fullness in the involved ear. Over the past 4 decades, the Equilibrium Committee of the AAO-HNS has issued guidelines for diagnostic criteria, with the latest version being published in 1995. These criteria were reviewed in 2015 by the Equilibrium Committee, and revisions were approved at the recent meeting of the committee at the 2015 AAO-HNSF Annual Meeting. The following commentary outlines the amended and approved criteria. (Source: Otolaryngology - Head and Neck Surgery)

Systematic reviews and meta‐analyses represent the uppermost ladders in the hierarchy of evidence. Systematic reviews/meta‐analyses suggest preliminary or satisfactory clinical evidence for agnus castus (Vitex agnus castus) for premenstrual complaints, flaxseed (Linum usitatissimum) for hypertension, feverfew (Tanacetum partenium) for migraine prevention, ginger (Zingiber officinalis) for pregnancy‐induced nausea, ginseng (Panax ginseng) for improving fasting glucose levels as well as phytoestrogens and St John’s wort (Hypericum perforatum) for the relief of some symptoms in menopause. However, firm conclusions of efficacy cannot be generally drawn. On the other hand, inconclusive evidence of efficacy or contradictory results have been reported for Aloe vera in the treatment of psori…

Conclusion A proportion of tinnitus sufferers with self-professed normal hearing are likely to have mildly elevated pure tone audiometric thresholds. In patients with unilateral tinnitus, such elevated pure tone hearing thresholds are likely to be in the tinnitus ear and the contralateral non-tinnitus ear.[…]Thieme Publicações Ltda Rio de Janeiro, BrazilArticle in Thieme eJournals:Table of contents | Abstract | open access Full text (Source: International Archives of Otorhinolaryngology)

We describe the case of a patient who presented with debilitating pulsatile tinnitus in association with two jugular bulb diverticula. The diverticula were treated with stenting of the jugular bulb and coil embolization of the diverticula over two procedures. This resulted in successful resolution of symptoms and at 10 months follow-up the patient is asymptomatic. The technique is discussed with regard to similar published cases and surrogate measures of safety taken from the literature pertaining to idiopathic intracranial hypertension. Conclusions Vestibular migraine is one of the leading causes of dizziness in both adults and children. A high percentage of patients showed abnormalities in Videonystagmography during testing documenting vestibular system involvement in migraine patients during the dizziness attack. Good history taking and analysis of aural symptoms helps in the differential diagnosis. The profile of VM patients is outlined. (Source: Egyptian Journal of Ear, Nose, Throat and Allied Sciences)

Discussion This case report describes the physical therapy management of an individual with NTS. The management strategy followed a protocol similar to that used for cervicogenic headaches (CH) due to the involvement of the upper cervical spine with both NTS and CH and the lack of evidence for the treatment of NTS. This article presents a case of three war veterans with PTSD symptoms who received a series of acupuncture treatments for tinnitus with positive outcomes. Even though the article presents cases of only three veterans and was based on self-reports, there were very clear trends on how veterans with tinnitus symptoms responded to acupuncture treatments. Information generated from this case presentation is a good starting place in exploring evidence-based approaches in treating tinnitus symptoms in war veterans with PTSD. Publication date: Available online 4 February 2016 Source:NeuroImage: Clinical Author(s): Anusha Mohan, Dirk De Ridder, Sven Vanneste Brain networks are small-world networks typically characterized by the presence of hubs, i.e. nodes that have significantly greater number of links in comparison to other nodes in the network. These hubs act as short cuts in the network and promote long-distance connectivity. Long-distance connections increase the efficiency of information transfer but also increase the cost of the network. Brain disorders are associated with an altered brain connectome which reflects either as a complete change in the network topology, as in, the replacement of hubs or as an alteration in the connectivity between the hubs while retaining network structure. The current …

Low level laser therapy (LLLT) was first introduced for potential clinical applications by Mester in the late 1960s. This form of laser treatment, currently referred to as photobiomodulation (PBM), limits radiation intensity by transferring low energy to tissues and thereby does not generate heat. Among the clinical indications for which LLLT has been reported to be efficacious are pain relief (back and neck, orthodontic, shoulder), wound healing, carpal tunnel syndrome, colorectal cancer, elbow disorders, fibromyalgia, lymphedema, musculoskeletal dysfunction, myofascial pain syndrome, neurological dysfunctions, patella-femoral pain syndrome, rheumatoid arthritis, shoulder impingement syndrome, and tinnitus. Conclusions In patients who complained of subjective nonpulsatile tinnitus with unknown etiology, some temporal bone vascular variations, including high riding jugular bulb, dehiscent carotid canal, left-sided MEV enlargement, and petrous bone pneumatization, seemed to have an association with tinnitus. Further studies comparing all these entities between pulsatile and nonpulsatile groups and healthy controls should be undertaken. (Source: Surgical and Radiologic Anatomy)
