The Zen of Tinnitus Acceptance

In the 1980s I absolutely loved punk rock. I lived in Greensboro, N.C. and struggling punker bands traveling from Atlanta to D.C. for weekend gigs would unload their equipment-filled vans into tiny bars for mid-week performances to pay for gas money for the trip. I would move toward the stage to listen to the raging of tatted, bare-chested men, roaring like aircraft engines, their words almost impossible to decipher. You didn’t need to. You didn’t listen to this music. You felt it. Now in my late ‘60s guess what I get? Can you say “tinnitus”? When it first showed up a decade or so ago I began wearing earplugs, closing doors quietly, and avoiding loud noises in the hopes that at least I could prevent further damage and perhaps turn down the volume a bit. They have the advantage of being minimally invasive and may selectively eliminate the site of turbulence. Neuroscience of Tinnitus

Tinnitus is a consequence of changes in auditory and nonauditory neural networks following damage to the cochlea. Homeostatic compensatory mechanisms occur after hearing loss and these mechanisms alter the balance of excitatory and inhibitory neurotransmitters. In many individuals with hearing loss, chronic tinnitus and related phenomena emerge. Some people with tinnitus are disturbed by this subjective sensation. When auditory network dysfunction is coupled with limbic-gating dysfunction, an otherwise meaningless auditory percept such as tinnitus may acquire negative emotional features. What Is That Noise!?! My Take On Living With Tinnitus

Do you hear that? I often ask my family that question. Sometimes I can’t tell if the noise I am hearing is my tinnitus or if the sound is actually there. Usually, it is all in my head. Typically, I know this, to be honest, but I ask anyway, just in case. I have a 40-50% hearing loss in both of my ears, but only mild tinnitus. I am grateful for that. Sometimes I feel that the tinnitus is worse than the hearing loss! Unexplained sounds buzzing and whistling in your head can make you question your sanity. And give you the worst headache known to man. The lack of sound seems almost a relief in retrospect. Thus far, I have experienced two types of tinnitus. Abstract Increases in the acoustic startle response (ASR) of animals have been reported following experimental manipulations to induce tinnitus, an auditory disorder defined by phantom perception of sound. The increases in ASR have been proposed to signify the development of hyperacusis, a clinical condition defined by intolerance of normally tolerable sound levels. Tinnitus prevalence in New Zealand.

CONCLUSION: This is the first nationally representative study of tinnitus prevalence in New Zealand and largest study sample internationally for tinnitus prevalence to date. Long-term vestibulocochlear functional outcome following retro-sigmoid approach to resection of vestibular schwannoma

The objective of this study was to evaluate long-term vestibulocochlear functional outcomes of patients operated for unilateral vestibular schwannoma via the retro-sigmoid approach. Patients who underwent vestibular schwannoma resection via retro-sigmoid approach between 2004 and 2008 at our institution, without prior surgical or radio-surgical therapy were considered to be eligible for this study. Endovascular treatment of jugular bulb diverticula causing debilitating pulsatile tinnitus

We describe the case of a patient who presented with debilitating pulsatile tinnitus in association with two jugular bulb diverticula. The diverticula were treated with stenting of the jugular bulb and coil embolization of the diverticula over two procedures. This resulted in successful resolution of symptoms and at 10 months follow-up the patient is asymptomatic. The technique is discussed with regard to similar published cases and surrogate measures of safety taken from the literature pertaining to idiopathic intracranial hypertension. This article presents a case of three war veterans with PTSD symptoms who received a series of acupuncture treatments for tinnitus with positive outcomes. Even though the article presents cases of only three veterans and was based on self-reports, there were very clear trends on how veterans with tinnitus symptoms responded to acupuncture treatments. Altered top-down cognitive control and auditory processing in tinnitus: evidences from auditory and visual spatial stroop.

Attentional Bias in Patients with Decompensated Tinnitus: Prima Facie Evidence from Event-Related Potentials

In this study, we adopted an objective indicator of attentional processing, i.e. the mismatch negativity (MMN), to assess the attentional bias in patients with decompensated tinnitus. Three kinds of pure tones, D1 (8,000 Hz), S (8,500 Hz) and D2 (9,000 Hz), were used to induce event-related potentials (ERPs) in the normal ear. Surgical management of vestibular schwannomas after failed radiation treatment

Malignant Jugular Paraganglioma: Unusual Presentation on 68Ga DOTANOC PET/CT

We report a 40-year-old woman of jugular paraganglioma, presenting with right-sided neck swelling, hearing loss, and pulsatile tinnitus. Contrast-enhanced CT temporal bone revealed a mass in the right jugular foramina and extending inferiorly to internal jugular vein. 68Ga DOTANOC PET/CT was performed, which revealed somatostatin receptor expressing lesion in the right internal jugular vein and extension into sigmoid sinus and additional metastatic focus in the sacrum.
